If you are reading this, you have probably agreed to tutor one or more of our ISDD clients. ISDD — Innovative Solutions for Disadvantage and Disability — is funded by various grants and needs to report back on the benefits their clients are receiving. Starting in November 2023, In-Home Tutors asks tutors to create Service Plans for each child, to allow us to track their progress over the course of the authorized tutoring period.
First, read the guidelines on how to create the Service Plan.
Now, if you’ve read the guidelines, you’re ready to create your plan.
Step-by-Step Guide to Completing the Service Plan
- Access the Service Plan form via any browser at this link.
- Put in the student’s name (separate form are needed for each child in the family), then your name and email
- Add 3-5 specific, measurable goals
- Ignore the Results section for now.
- Click Submit
- Download a copy that you can refer back to as tutoring continues over the next few months.
- VERY important: you will receive an email with the link to the survey. Save the email (or click on the link and bookmark it your browser). You will need to return to it to complete the results of your goals.
- We will email you when it’s time to return to your form. You’ll find your form link and click on the Edit You Form option. Then click the + sign to add the results of your goals.
